My Aged Care Process:
Your Guide to Accessing Support

Most Australians value their independence and want to remain in control of decisions about how they live their lives, even when facing health or mobility challenges. My Aged Care is the starting point for accessing Australian Government-funded aged care support, helping you stay independent and well-supported at home.

Who is Eligible for My Aged Care?

My Aged Care is available to individuals who meet the following criteria:

Age Requirements: People aged 65 and over (or Indigenous Australians aged 50 and over).

Support Needs: Those requiring additional assistance due to:

  • A recent fall, reduced mobility, or changes in memory.
  • A diagnosis of a medical condition.
  • Changes in existing care arrangements.

Depending on your eligibility, the Government may partially or fully fund your care needs. You can check your eligibility and apply for an assessment on the My Aged Care website or by calling 1800 200 422.

Steps to Access My Aged Care Services

The My Aged Care journey is straight forward, with three key steps to ensure you receive the support you need:

STEP 1: Get Assessed

To access My Aged Care funding, an assessment is required.

This involves:

  • Registering with My Aged Care via their website or by calling 1800 200 422.
  • My Aged Care will arrange an in-home visit from an assessor to evaluate your care needs and determine your eligibility for government-funded support.

Tip: Be prepared to discuss your daily challenges and the types of support that would help you most.

STEP 2: Choose a Care Provider

Once approved, you’ll need to select a provider to deliver your care. Providers act as fund managers, ensuring your services are arranged and paid for within your funding package.

How to Choose a Provider:

  • Use the “Find a Provider” tool on the My Aged Care website.
  • Ask your assessor for referrals.
  • Speak with providers directly to compare services and costs.

STEP 3: Manage Your Services

Once you’ve chosen a provider, your care can begin. While self-management is available through some providers, many people prefer to have their Support At Home Package managed for them. A managed package means less stress and paperwork, with your provider handling budgets, service coordination, and compliance. It ensures your care is professionally overseen, tailored to your needs, and that you’re supported every step of the way.
