Everything You Should Know About the New Support at Home Program
From 1 July 2025, the Australian Government will replace the Home Care Packages Program and Short-Term Restorative Care Programme with the Support at Home program. For those on the Commonwealth Home Support Programme (CHSP), the transition will occur no earlier than 1 July 2027. These updates aim to make aged care simpler, more equitable, and better at supporting older Australians to maintain their independence at home.

What Is the Support at Home Program?
The Support at Home program combines several aged care initiatives into one cohesive system. It focuses on offering tailored care, enhancing flexibility, and improving access to essential services.
Key Features & Benefits
- Increased Funding Limits:
Annual funding for care at home is rising to $78,000, providing enhanced support for participants with greater care needs. - Supportive Home Modifications & Equipment:
Participants may access $15,000 annually to purchase aids like walkers, shower chairs, and grab bars or for home changes like ramps and handrails that boost safety and independence. - Fair Cost Contributions:
Fees will depend on your financial situation:- The Government will fully cover clinical care such as nursing.
- Contributions for daily living and independence services will be capped and proportionate to your means.
- Specialist Pathways for Complex Needs:
New options include the Restorative Care Pathway for rehabilitation and the End-of-Life Pathway, which offers up to $25,000 for extra care over three months. - Extended Short-Term Care Options:
Restorative care now lasts up to 12 weeks, with extra time available in some cases. These programs focus on allied health and goal-based rehabilitation.
Specialised Pathways
- Restorative Care Pathway:
Designed to help participants regain strength and independence, this pathway provides coordinated allied health services for up to 16 weeks in some cases. It’s ideal for those needing temporary, intensive care. - End-of-Life Pathway:
This brand-new option offers significant funding and resources to help those in their final months of life stay at home with dignity. It works alongside state-based palliative care services, covering up to $25,000 over three months.
For Existing Home Care Recipients
- Automatic Transition:
If you’re currently using a Home Care Package, you’ll switch over to the new program without needing a reassessment, and your existing funding levels will remain intact. - Carry-Over of Funds:
Any unspent money from your current Home Care Package will transfer, ensuring you maintain access to essential services.
For Those New to Home Care
- Streamlined Approvals:
The Single Assessment System will simplify the process, with a target to reduce wait times to three months by 2027. - Customised Support Plans:
You’ll receive a detailed plan tailored to your needs, outlining services, budgets, and goals.
Continuity of Care
- Provider Retention:
If your current provider is registered with Support at Home, they’ll continue offering your services without interruption. - Care Management Remains Key:
Care managers will guide you through the system, adjusting your care plan as your needs change. A dedicated portion of your budget will ensure you continue receiving this vital support.
