Helping You Make Informed Choices About Funding and Future Care Needs

Join us for a supportive and informative session led by our General Manager, David Fleming. This event is designed to help you better understand additional funding pathways available under Support at Home — including End-of-Life Pathways — so you and your family can feel informed, reassured, and prepared for future care needs.

What You’ll Learn

• Understanding additional funding pathways – what they are, who they may apply to, and how they can support changing care needs over time.

• End-of-Life Pathways (included) – what support may be available when someone’s care needs increase, and how planning ahead can reduce stress for clients and families.

• Advanced care planning – documenting wishes, planning ahead, and choosing an Enduring Guardian.

• Support for families and carers – how families can be supported through decision-making, conversations, and planning.

• Emotional, social and practical support – support available through HCA and partners, including social work and nursing guidance.

• Who can help and how to access support – the role of nurses and social workers in navigating funding pathways and connecting you with the right support at the right time.
